Visitors moving into the area are welcome to register at this surgery.
If you do not have a medical card you will be asked to complete a registration
form giving your name, address, date of birth, and details of your previous
address and doctor. This allows the Health Authority to arrange for
your medical record to be transferred to us.
If you do have a medical card, please bring it with you to the surgery
when you want to register as this shows all the information required.
If you would like to be registered with a specific doctor please tell
the receptionist when you register. Unfortunately, some of the doctor's
lists are not open to new patients but, wherever possible, your choice
will be taken into consideration.
When you register with the practice you will be invited to make an appointment
with the practice nurse for a new patient health check. This allows
any significant health problems or family history to be recorded prior
to the receipt of your medical record and allows you to discuss any
issues or concerns you may have.
You
will also be given a practice leaflet which gives you all the information
about the services provided by the practice and the surgery opening
times
It is the ethos of this practice to encourage continuity of care and
the concept of your own personal doctor.
When you register with the practice you will be allocated to one of
the two partners who will be your named doctor and be responsible for
overseeing your care, although you may see the other doctors, when you
attend the practice (such as Locums or Registrars)
